What are PMADs?

Perinatal mood and anxiety disorders (PMADs) are mental health conditions — including postpartum depression, perinatal anxiety, and perinatal OCD — that can develop during pregnancy or in the year following birth, affecting up to 1 in 5 pregnant or postpartum people. They are among the most common complications of the perinatal period, yet remain widely undertreated.

The two most common are postpartum depression, marked by persistent sadness or emptiness that goes beyond the normal adjustment of new parenthood, and perinatal anxiety, which involves excessive worry or fear during pregnancy or after birth.

PMADs can affect anyone, regardless of age, background, or prior mental health history. They are not a sign of weakness or poor parenting but treatable medical conditions.

What to look for

What are the symptoms of PMADs?

PMADs can look different in each person. The most common symptoms are:

Emotional

  • Persistent sadness, emptiness, or emotional numbness
  • Intense anxiety, worry, or a sense of dread that is difficult to control
  • Irritability, anger, or feeling easily overwhelmed
  • Guilt, shame, or feeling like a bad parent

Cognitive

  • Intrusive thoughts about harming the baby or oneself (this is a well-documented symptom of PMADs — having these thoughts does not mean you will act on them)
  • Difficulty concentrating or making decisions
  • Persistent negative thoughts about one's ability to cope or parent

Physical

  • Changes in sleep beyond what a newborn demands, including inability to sleep even when the baby sleeps
  • Changes in appetite or significant weight changes
  • Fatigue that goes beyond normal new-parent exhaustion

Behavioral

  • Withdrawing from the baby, partner, family, or friends
  • Excessive checking behaviors related to the baby's safety
  • Avoiding situations or people out of fear or anxiety

How are PMADs diagnosed?

A clinical evaluation typically involves a conversation about your symptoms, medical history, and your experiences during pregnancy or the postpartum period and may include validated screening tools like the Edinburgh Postnatal Depression Scale (EPDS).
